- comments: observed the handwashing sinks in the toilet rooms on site without handwashing signage. Instructed to provide and maintain.

- comments: observed improper temperature of tcs foods such
raw chicken leg quarters (56. 1f-62. 5f), beans (48. 9f), cooked goat (60. 6f-63. 5f), cooked chicken stew (65. 3f), fish fillets (48. 5f-50. 6f). Manager discarded product. Instructed must have cold holding foods at 41. 0f or below approx. 300lbs at $1,600 discarded. Priority 7-38-005, citation issued

- comments: observed juices (ginger beer and homemade juice) prepared and bottled on site stored at the front self-service refrigerator with no warning label on the bottles. Instructed manager to provide haccp or warning label for prepackaged juices. Priority foundation violation 7-38-005, citation issued

- comments: observed no probe stem thermometer for taking food temperatures. Must provide. Priority foundation 7-38-005, citation issued.

- comments: prepackaged beverages for self-service at the display cooler at the front of the facility. Not properly labeled. Must label packages with food name, ingredients, net contents, a completed and list of allergens if any are used.

- comments: observed the front and two rear security gates not rodent proof while the doors were open. Instructed to provide a screen on all security gates.

- comments: observed evidence of pest activity. Observed 25-40 small flies and 2 flies in the rear of food prep area coming from under the three compartment sink, coming from the white buckets on the floor near the handwashing sink in the rear food prep area, in the rear toilet room near the food prep area, the inside of the three door refrigerator at the rear of the facility. The 2 flies were observed in thre rear food prep areas. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a).
44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed the bowls and pans not inverted in the rear food prep are on the shelving unit under the food prep tables. Instructed to invert said kitchenware in the said areas and maintain.
49. Non-food/food contact surfaces clean
- comments: observed the interior of the oven with grease spills and grease droplets. Instructed to clean said equipment and maintain.
51. Plumbing installed; proper backflow devices
- comments: observed a leak from the pipe in the employee toilet room near the cold holding units at the rear. Instructed to repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed the floors throughout the entire the rear food prep area and cold holding area especially behind and under the equipment in both areas with food debris and not maintained. Instructed to clean and maintain said floors.
60. Previous core violation corrected
- comments: previous core violations not corrected from 06/03/2021, report #2510772 #47: observed build up of dust on the wall and side of convection oven in the rear food prep area. Instructed to clean, sanitize and maintain. #47: observed no splash guard at rear hand washing sink next to table where foods are prepared. Instructed to provide and maintain as a means of preventing possible contamination of any foods near the hand washing sink. #56:observed dust build up on hood ventilation system in food service area and missing ventilation covers on wall. Instructed to clean vents and replace missing vents in facility prep area. #57 observed no food handlers certification on site. Instructed that all employees outside of city of chicago food service manager must obtain and provide on site. #58: observed no allergen certification for city of chicago food service manager on site. Informed manager that all certified city of chicago managers must obtain and provide on site. Priority foundation:7-42-090, citation issued
