Had a burrito bowl w/ beans rice avocado and veggies. Had lunch around 1:00 PM on 6/15/19 and felt nauseous around 8:00 PM. Nausea has continued for a day followed by bad diarrhea.

ate chipotle burrito bowl, Im the only member of my family that ate chipotle I ate it on Monday and was vomitting and diarrhea with fever until today
