I ate at Mod Pizza January 14th 2018 at about 3pm. Also Mod Pizza is the only food I consumed this day. I had a regular crust 11inch with pepperoni, spicy sausage, and roasted mushrooms. I began feeling ill (nauseous, stomach churning) later that evening around 6pm.…
