2. City of chicago food service sanitation certificate
- comments: observed no certified food manager with the original city of chicago food service manager certificate on site, while open and operating. Instructed a city of chicago certified food manager must be on site with the original city of chicago food service manager certificate posted at all times while foods are being prepared and served. Priority foundation violation 7-38-012, citation issued

22. Proper cold holding temperatures
- comments: observed improper cold holding temperatures of tcs foods. Found approximately 10 packages of raw pork tenderloins (65. 9f-68. 0f) in the three compartment sink with no water just ice cubes, eggs (60. 8f) and fried tofu (48. 2f) in the prep cooler, fried crab rangoon (48. 9f-50. 0f) stored inside another prep cooler. Instructed must have cold holding foods at 41. 0f or below. Approximately 144lbs at $199 worth of product discarded. Priority 7-38-005, citation issued

35. Approved thawing methods used
- comments: observed improper thawing methods for tcs foods. Found packages of pork tenderloins in the three compartment sink with not water just a few ice cubes. Instructed to use proper thawing methods such as placing food in the walk-in cooler overnight, or under cold running water.

38. Insects, rodents, & animals not present
- comments: observed approximately 15-20 rat droppings observed on site. Found on floors in the followingareas: in the rear hot water tank closet, surrounding the dishwashing equipment in the rear, and under the cooking equipment under the ventilation hood. Instructed to remove rat droppings, clean and sanitize all affected surfaces. Priority foundation violation 7-38-020(a), citation issued

55. Physical facilities installed, maintained & clean
- comments: observed a hole in the lower wall behind the hot water tank in the hot water tank storage closet in the rear. Instructed to seal mentioned hole to eliminate potential harborage points for pest.

58. Allergen training as required
- comments: observed no allergen from the food manager with the servsafe certificate on stie at this time. Instructed all food managers must take course and maintain record on site.

60. Previous core violation corrected
- comments: observed previous core violations from report #2618629, date06/04/2025: violation not corrected. #38: rear screen door with 1/2"-1" gaps underneath and is not rodent/insect proofed. Instructed to rodent/insect proof door or close the outer door. Maintain same. #39: tcs foods stored in grocery bags and other non-food gradeable bags. Instructed to store in food gradeable bags and maintain. #47: storage racks inside the walk in cooler with rust. Instructed to remove rust or replace racks and maintain rust free surfaces. #49: lower surfaces of all coolers/freezers with food and dirt. Storage racks inside the walk in cooler/freezer with food and dirt. Instructed to clean and maintain all. #51: the pipe under the front handwashing sink and the base of the right faucet on the 3-compartment sink leaks. Instructed to fix and maintain all leaks. #55: the floors in the corners, under/around the coolers/freezers, storage racks, cooking equipment and inside the walk in cooler/reezer with food, dirt and debris. Instructed to clean and maintain floors. -food splatter on the ceiling above the cooking equipment. Instructed to clean and maintain. Instructed manager to correct all the above violations. Priority foundation violation # 7-42-090 citation issued.
