- comments: found no employee health policy at premises. Priority foundation. 7-38-010. Must provide.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no soap and paper towels at hand washing sink in rear. Also observed no soap in employee bathroom. Must provide soap and paper towels at hand washing sink at all times for employees to properly wash hands correctly. Priority foundation. 7-38-030(c).

- comments: observed no employee hand washing sign at hand washing sink in employee bathroom and in rear near 3-compartment sink. Must be posted at all times.

16. Food-contact surfaces: cleaned & sanitized
- comments: observed no sanitizer on site during inspection that is used to clean equipment and utensils. Instructed manager, sanitizer must be provided and available for use during all hours of operation. Manager went to store to pick up bleach. Priority violation. 7-38-005.

- comments: observed bulk containers not labeled (dry peas & rice). Must label with common name on food item container to properly identify.

- comments: observed no chemical test strips or test kit on site that accurately measures the concentration of the sanitizer solution. Must provide on site at all times. Priority foundation. 7-38-005.

- comments: observed inside of cooking fryers filled with heavy grease build up. Must be detail cleaned and maintained.

- comments: observed employee bathroom door not properly closing. Must provide self-closing device on door to properly close at all times.

- comments: observed heavy water damage on ceiling tiles in employee bathroom. Must replace.

- comments: observed ventilation hood accumilated with extremely heavy grease build up. Must be serviced, cleaned and maintained properly to prevent contamination.

- comments: observed no food handler training on site during inspection. Instructed to provide food handler training for all current employees.

- comments: food sanitation manager with no allergen training. Must obtain on site at all times.
