16. Food-contact surfaces: cleaned & sanitized
- comments: we observed black and pink slime substance at interior of ice machine water reservoir in prep area. Ice is used for human consumption. Instructed to clean, sanitize and maintain unit. Priority foundation violation:7-38-005,citation issued

28. Toxic substances properly identified, stored, & used
- comments: observed a white gallon stored on floor without no labeling of toxic contents. Employee stated that was bleach in it. Instructed to properly label all working containers of toxic materials with its common name. Manager removed container upon my request priority foundation violation 7-38-015. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: two door reach-in cooler air temp of 58. 5f to 64f. Manager removed tcs food in the morning. Tagged unit:held for inspection' do not use'. Priority violation:7-38-005,no citation issued.

41. Wiping cloths: properly used & stored
- comments: all wiping cloths in all prep areas must be held in a container with a sanitizing solution between use.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must not use milk crates as a means of elevation for stored items.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: quaternary chemical test strips on site, however unable to check sanitizing levels for three compartment sink and sanitizer buckets to sanitize dining tables due to said strips at one point were wet and dried. Must provide chemical test strips to be able to check sanitizer solution. Priority foundation violation 7-38-005 citation issued.

55. Physical facilities installed, maintained & clean
- comments: mop sink room is extremely clutter,instructed to remove what is not needed and elevate 6'off the floor what is needed

60. Previous core violation corrected
- comments: previous core violation not corrected from 4-23-2019,#2285649 #51) 5-204. 12 unable to identify back flow prevention/vacuum breaker device on: ice machine in rear(unit now is relocated at front under service counter). Instructed manager to identify and or install back flow prevention/vacuum breaker device on said areas. Must install so back flow prevention device may be located to be serviced and maintain. Priority foundation violation:7-42-090,citation issued.

61. Summary report displayed and visible to the public
- comments: previous inspection summary report from april 23,2019 report# 2285649 not posted conspicuous to consumers as required nor on site. Priority foundation: 7-42-010(b). New summary report given and posted
