- comments: observed pic on duty at this time has no city of chicago food sanitation certificate when tcs foods are being prepared, and served, ( chicken,rice, etc). Priority foundation issued 7-38-012
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: repeat-observed no employee health policy for employees health available. Instructed manager to provide. Priority foundation violation issued 7-38-010
39. Contamination prevented during food preparation, storage & display
- comments: observed whie cuuting board with some dark grooves in prep area. Instructed to repair or replace.
49. Non-food/food contact surfaces clean
- comments: observed bottom shelves of prep tables storage shelving units with debris, dirt. Must clean/maintain. Dust
51. Plumbing installed; proper backflow devices
- comments: observed drainpipe leaking under rear hand washing sink. Instructed manager to repair and maintain sink
54. Garbage & refuse properly disposed; facilities maintained
- comments: outside garbage area not maintained. Observed dumpster lids in poor repair,( bent, not able to cover and close on right side and the middle lids pushed inside of dumpster. Instructed to replace lids to fit/ cover garbage dumpster. Priority foundation issued 7-38-020(b).
58. Allergen training as required
- comments: instructed that managers with city of chicago food sanitation certificate must have allergen training. Must provide.
