- comments: observed front entrance door of premise with large gap on bottom between door and floor, also rear exit door leading to outside receptacle open during time of inspection. Instructed to enclose in order to prevent pest entry.

- comments: observed approximaltely 20 large rodent droppings on premise behind tv on floor in sitting room, in office in rear, in pantry space under shelving units along the wall, in storage room behind heavy equipment, and throughout office space connected to seating area. Instructed to remove droppings in all said areas, clean and sanitize. Noted approximately 30 small live flies flying throughout rear storage area, kitchen space for employees and both toilet rooms. Must eliminate pest activity. Priority foundation violation. #7-38-020 (a). No citation issued.

- comments: observed large holes in wall near entry door for pantry, large gaps along baseboard in sitting area on north and south sides of wall and in small office space attached to rear office with holes along wall. Instructed to enclose large spaces to prevent pest entry to premise.
