- comments: 2-102. 14(o) observed no written employee health policy on the premises. Management instructed that to have a signed employee health policy for all employees. Priority foundation violation 7-38-010.

- comments: 6-301. 14 must provide hand washing signage at all hand washing sinks including the washrooms and behind the bar.

38. Insects, rodents, & animals not present
- comments: 6-202. 15 observed a 1/2" gap at the bottom corner of the rear exit door and the bottom of the front door. Management instructed to rodent proof all outer openings to prevent rodent entry and harborage conditions.

- comments: 5-204. 12 no backflow prevention device observed at the water supply line to the ice machine. Management instructed to install.

- comments: 6-501. 114 remove all excessive clutter from the basement storage areas that is contributing to rodent harborage conditions.

- comments: 8-404. 13(b)(3) previous priority foundation violations from report 2630092 on 1/21/26 not corrected: 38 - observed over 100 rat droppings on top of the wooden liquor cabinet and walk-in beer keg cooler, behind the ice machine and sliding door beer cooler, on the floor and on top of the electrical boxes in the back boiler room. Management instructed to clean and remove all rat droppings from all affected areas. Also observed approx. 10 live small flying insects flying around in the basement and breeding in the plastic tubs used to collect water leaks. Additional pest control service is needed to eliminate the pest activity. 60 - previous core violations from report 2598545 on 8/2/24 not corrected: 55 - noted large holes/opening in the floor under the bar. Instructed to seal all opens to be smooth and cleanable. Remove spray foam insulation and seal the holes properly. Noted dirt and debris under the bar, on floors of the d. J. Booth, throughout the basement storage, behind and around ice machine. Also noted pooling liquid throughout the rear of the basement. Instructed to clean all, remove all pooling liquid and maintain dry floors. No ceiling above the basement ice machine. Must provide a smooth cleanable ceiling above the ice machine where it is dispensed. Priority violation 7-42-090.
