- comments: observed approximately 8 live roaches located in the following areas on wall behind handwashing sink and on shelving unit below microwave in cash register area. Instructed pic have pest control operator service premises before 03/20/2025. Priority foundation violation #7-38-020(a). Citation issued.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed men's and women's toilet room doors in bar area to not be self closing. Instructed to install a self closing device and maintain at all times
55. Physical facilities installed, maintained & clean
- comments: observed mops (2) in a position that does not allow to dry properly and prevent pest harborage. Must store mop after use in a mounted position that allows for air-drying without soiling and pest harboraging.
