- comments: observed no verifiable health policy on site at time of inspection. Left template and instructed to maintain copies of health policy signed by all employees on site at all times. Priority foundation violation 7-38-010. No citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no kit and procedure on site for responding to vomit and diarrhea. Left one pager and instructed to maintain procedure and kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005.

38. Insects, rodents, & animals not present
- comments: observed one live roach near main cook line, one dead roach near cook line, and approximately 40 roach droppings underneath 3 compartment sink, behind water heater, and near employee restroom. Instructed to receive additional pest control service from licensed professional and maintain facility free of pests. Priority foundation violation 7-38-020(a). Citation issued.

- comments: observed rear door does not shut tightly creating gap between door and frame. Instructed to repair or replace to create a firm seal in order to prevent entry of pests.

- comments: observed cutting boards in prep area covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable surfaces.

- comments: observed slight leak on pipes underneath exposed hand sink and loose and leaking faucet on 3 compartment sink. Instructed to repair or replace and maintain in good condition.

- comments: observed heavy build up of grease, grime, and food debris on floors and baseboards of prep area. Instructed to detail clean and maintain in good condition.
