- comments: observed no city of chicago certified food manager on site with valid original certificate while preparing and serving tcs foods (chicken, fish sandwiches, mashed potatoes). Instructed to provide only on-site all times while open and operating. Priority foundation violation. #7-38-012. Citation issued.
21. Proper hot holding temperatures
- comments: observed approximately 6lbs of cooked chicken pieces at improper temperature located on the table in the prep area during the time of inspection. Noted temperatures ranging between 104. 5f -123. 0f. Instructed to hold all hot time/temperature control for safety foods at 135f or higher as required. Priority violation. #7-38-005. Citation issued. Said food was voluntarily discarded at this time by the person in charge.

- comments: observed missing thermometer inside condiment and cole slaw cooling units. Instructed to provide.

- comments: observed the inside of the deep fryer (rear) oil filtering unit with a build-up of grease and food debris. Noted excess food debris on all surfaces of heavy equipment throughout the prep area. Instructed to clean and maintain.

- comments: observed broken wall base throughout walk-in cooler as well as dishwashing area near mop sink. Instructed to enclose to prevent pest harborage.
