Details OBSERVED THE INTERIOR OF THE ICE MACHINE'S UPPER RESERVOIR WHERE ICE FOR CONSUMPTION IS MADE WITH A PINK SLIME SUBSTANCE DRIPPING DOWN INTO THE READY TO EAT ICE. ALSO OBSERVED SAME PINK SLIME SUBSTANCE ON THE INTERIOR OF THE PLASTIC ICE SHOOT OF THE SODA DISPENSING SYSTEM IN THE DINING ROOM. INSTRUCTED TO CLEAN AND SANITIZE ALL EFFECTED AREAS INSIDE THE MACHINE AND THE ICE SHOOT. REMOVE ALL SLIME. PRIORITY FOUNDATION VIOLATION 7-38-005 CITATION ISSUED.
OBSERVED INACCURATE INTERNAL THERMOMETER IN THE EGG SANDWICH PREP COOLER. MUST REPLACE.
ALL BULK FOOD CONTAINERS MUST BE LABELED WITH THE PRODUCT COMMON NAME.
ALL PREP AREA WET WIPING CLOTHS MUST BE HELD IN A CONTAINER WITH A SANITIZING SOLUTION BETWEEN USE.
PLASTIC SHELVING THROUGHOUT REAR PREP DIRTY WITH FOOD DEBRIS. OBSERVED DIRTY BULK FOOD CONTAINERS. MUST CLEAN AND MAINTAIN ALL.
BROKEN/DAMAGED TILES AT THE MOP SINK. MUST REPLACE/REPAIR.
OPEN GAP AROUND PIPING UNDER THE THREE COMPARTMENT SINK. MUST CAP/SEAL.----EXCESSIVE DUST IN CEILING VENTS IN BOTH WASHROOMS. DIRTY FLOOR DRAIN UNDER THE DISH MACHINE. MUST CLEAN AND MAINTAIN ALL.

Friday, April 12th, at around 10am I ordered a bacon egg and Gouda sandwich as well as a caramel ribbon drink. By around 3-4pm, I started feeling unwell. At 5 pm I vomited for the first time.
