- comments: observed rear exit door with 1/4' gap or more underneath. Instructed manager to rodent proof door and seal gap to prevent entry of unwanted pests.

55. Physical facilities installed, maintained & clean
- comments: observed clutter on second floor of rear storage /receiving dock area. Instructed manager to remove all unnecessary items to prevent pest harborage.

- comments: during reinspection, observed water stained ceiling tiles throughout the facility. Observed a buildup of dust debris and grime under all shelving units on the main floor and in the rear storage areas. Instructed to detail clean, increase cleaning frequency, and maintain at all times.

59. Previous priority foundation violation corrected
- comments: during re-inspection observed continued non-compliance from report #2560773 dated 7/14/2022,observed over 100 rodent droppings in rear storage/dock receiving area along walls and underneath overstock, under shelving and end caps of ailes where product is stored. Instructed manager to contact pest control company for serice, remove droppings, clean and sanitize. Priority foundation. 7-38-020(a) citation issued.
