- comments: pic has no sanitation certificate.
2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago certified manager on duty while tcs foods are being prepared and handled such as chicken,fish and italian beef. Instructed manager to provide and maintain city sanitation certificate at all times. Priority foundation citation issued #7-38-012.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: **premises has no employee health policy. Instructed manager to comply with the new code or citations will follow. Priority foundation violation #7-38-010.
5. Procedures for responding to vomiting and diarrheal events
- comments: **premises has no clean up procedures or supplies for responding to vomiting and/or diarrhea events. Instructed manager to comply or citations will follow. Priority foundation violation #7-38-005.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the hand sink in the washroom broken/poor repair. Hot water knob broken can not turn on. Instructed manager to repair and maintain. Priority foundation citation issued #7-38-030(c).
49. Non-food/food contact surfaces clean
- comments: must defrost and clean the interior of the reach in freezer, also clean the interior of the reach in prep cooler to remove spills and debris.
51. Plumbing installed; proper backflow devices
- comments: observed a leaky pipe underneath the 3 compartment sink, also noted the hot water knob on the hand sink in the prep area in poor repair(very loose and hard to cut on). Must repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: floors need clean around, behind all heavy equipment to remove debris and/or grease.
57. All food employees have food handler training
- comments: instructed manager to comply with the food handler requirements for all food handler.
