- comments: observed the hand dryer in the customers ladies wash room not working and no paper towels provided for proper hand drying as required. Instructed manager to provide and maintain paper towels in the said washroom until hand dryer is repaired. Priority foundation citation issued #7-38-030(c)

55. Physical facilities installed, maintained & clean
- comments: floors need cleaning inside the walk in dairy and meat cooler to remove spills and debris.
