Report date: Nov 25, 202538. Insects, rodents, & animals not present
- comments: observed evidence of insect activity in the rear dish washing area approx 4 live roaches. . . The dish storage rack approx 5 live roaches. . . Front prep area 2 live roaches and several dead insects on rear throughout the facility. Instructed to clean and maintain areas as needed. Contact pest control service and have inspection and service before return date of december the 2nd. Priority foundation violation 7-38-020(b) citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed the lower shelving of all prep areas in the rear food service area in need of a detail cleaning. Instructed to remove all items, clean and maintain.
49. Non-food/food contact surfaces clean
- comments: observed the floors in all walk in cooling and freezer units in need of detail cleaning of dirt, food debris and debris. Instructed to clean and maintain all.

- comments: observed the sides of all cooking and non cooking equipment with a build up of grease,spillage or splatter between equipment as well. As well as a build up of dust, dirt and debris under the 10 burner unit in the rear kitchen area instructed to clean and maintain all areas.

- comments: observed the water knob under the sink attached to the sprayer hose of the dish washing machine area leaking. Instructed to repair and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed floors throughout rear food prep and dish washing area under and around cooking and non cooking equipment in need of a detail cleaning. Instructed to clean and maintain all areas as a means of preventing possible pest and or insect activity.
