- comments: observed frozen broccoli thawing on bottom of prep table. Instructed manager to properly thaw food in refrigerator, under cold running water or part of cooking process.

- comments: observed 5 live roaches crawling along basebaord under 3-compartment sink, 3 live roaches crawling on floor under dishmachine, 3 live roaches crawling on wheels of rolling prep table, 3 live roaches crawling on floor under rolling prep table, 2 live roaches on bottom shelf of rolling prep table. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed food items stored on floor in walk-in freezer. Instructed manager to elevate all items six inches off floor.
