- comments: observed accumulated food debris and grease buildup on interior and exterior surfaces of cooking eqipment, grill, fryer cabinets, holding units, slicer, prep tables, coolers, and freezers. Observed residue on interior panel of ice machine. Instructed manager to clean and maintain.

Observed dust buildup on light shields and ceiling vents in prep, storage, and dining areas. Instructed manager to clean and maintain.

- comments: previous priority foundatioin violation not corrected: 38-observed over 25 live roaches crawling on wall behind file cabinets in dry storage area, on floor along wall near hand sink in front prep area, inside compressor unit of prep cooler in front prep area, along crevices under prep table in rear prep area, along walls and floor in utility storage area.
