- comments: observed an unapproved insect control device hanging from the ceiling in the dry food storage area. Instructed to remove insect control device.

- comments: observed over 20 rat droppings and over 50 mice droppings scattered on the floor in the outside utensil storage unit. Instructed to remove droppings and clean and sanitize storage unit. Priority foundation violation. Citation issued. 7-38-020(a)

- comments: osbserved wet in use wiping cloths stored on the counters and prep tables throughout the facility. Instructed to keep in use wipe cloths in proper sanitizing solution.

- comments: observed single service foam trays stored face up at the cookline. Instructed to store all single service articles face down to prevent from contamination.

- comments: observed can opener in the prep area to be heaviliy soiled with food debris. Instructed to clean and maintain can opener.

- comments: observed spill food debris and soil accumulation on slicer, mixer, at the bottom of sandwich reach in cooler at the end cookline and at the bottom of two door reach-in freezer. Instructed to clean and maintain.
