3700 U.S. 41, Terre Haute, 47802 Indiana, United States

Ordered steak l, Mac and cheese and a blooming onion. Steak looked old and got replaced. Shortly after eating a few bites of the old steak and eating the Mac and cheese and blooming onion I got an awful stomach ache and to immedielty go to the bathroom. After leaving I had to stop somewhere else on the way home to go to the bathroom again and then more after I arrived home | Symptoms: Diarrhea, Nausea
