- comments: observed evidence of rodent activity on the premises. Observed approx. 30 mouse droppings on the floor and storage shelves inside of the storage room between the washrooms in the basement hallway. Also observed approx. 50 mouse droppings on the floor and on the storage shelves in the storage closet of room 024 in the annex building. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).
55. Physical facilities installed, maintained & clean
- comments: observed multiple broken floor tiles and holes along the walls of the lunchroom. Management instructed to repair all broken floor tiles and seal all holes in the floor to prevent rodent harborage conditions. Replace the missing floor tiles outside of the men's washroom in the cafeteria manager's office.
