- comments: observed no ambient air thermometer inside the refrigerator in the rear area with the three compartment sink. Instructed to provide and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed broken down cardboard boxes utilized behind the equipment and on the floor where the three compartment sink is stored, in the toilet room, and on the side of cold holding units. Instructed to remove and maintain.
