- comments: observed dead roaches on glue boards at dry storage area and behind refrigerators at food prep areas. Instructed management to remove and replace with new glue boards

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must clean and or repair all rusted shelving in walk in cooler unit

52. Sewage & waste water properly disposed
- comments: observed no grease traps at three compartment sinks. Instructed managemwnt to install grease traps at three compartment sinks

55. Physical facilities installed, maintained & clean
- comments: noted clutter of equipment and other unused items on floors in storage rooms. Must elevate all items to allow cleaning and remove all unnecessary items
