- comments: observed no hot water at hand washing sinks in second floor unisex washroom*75. 2f) first floor boys washroom south (86. 2f), secind floor boys washroom south (93. 2f), thrid floor boys washroom north (89. 4f), third floor girls washroom north (75. 2f). Instructed manager to provide at least 100f at all hand washing sinks. Priority 7-38-030(c)
38. Insects, rodents, & animals not present
- comments: observed over 60 mice droppings scattered on floor along walls behind grease trap, 3-compartment sink and cooking equipment, in corners in kitchen, 20 mice droppings scattered in corners of hallway leading to lunchroom, 20 mice droppings scattered in corners of lunchroom, 20 mice droppings scattered on shelves in dry storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed 1/4' gap on left corner of right entrance door. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed no covered waste receptacle in first, second, and third floor girls washrooms. Instructed manager to provide.

- comments: observed missing and loose baseboards along walls in kitchen and storage areas. Observed peeling paint on walls behind milk cooler, on ceiling in girls lower level washroom and staff second floor washroom. Observed openings around pipe fittings under sink in second floor staff washroom. Instructed manager to repair and maintain.

- comments: observed unused articles and equipment in unused lunchroom near kitchen. Instructed manager to remove all unnecessary items to prevent pest harborage.
