- comments: observed no clean-up of vomiting and diarrhea procedure and clean-up kit on premises during inspection. Instructed to have said procedure along with required equipment for clean-up. Priority foundation violation. No citation issued. 7-38-005

- comments: observed facility missing employee hand washing signage when returning to work from restroom. Informed person in charge to obtain and maintain signage

- comments: observed improper temperature of tcs foods such as eggs 47f and prepacked lunch meals 47. 9 and fruit juice 47 f. Product was discarded by manager. Must have cold holding foods at 41. 0f or below. (priority 7-38-005) no citation issued

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed improper temperature of refrigerator at 47. 9f with tcs foods stored inside for sale (eggs and prepacked lunch meals). A held for inspection tag has been placed on unit. - comments: noted no thermometers inside all refrigeration unit conspicuously posted to monitor the ambient temperature of equipment. Instructed to equip all refrigeration units with accurate and working thermometers

- comments: observed that pest control log book is missing required information. Missing copy of service ticket that states pest control idph license number or copy of new rentokil pest control company current idph license. Must provide and maintain all required pest control records and documents. Priority foundation violation. 7-38. 020 (c) no citation issued

- comments: must repair or replace broken hand soap soap dispenser in mans bathroom

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must clean and maintain clean plumbing fixtures in man's and lady's bathroom on 1st floor mostly used by clients.

55. Physical facilities installed, maintained & clean
- comments: observed disrepair of lady's room fixtures on 1st floor bathroom. And possibly water leak. Must repair and maintain and post temporary sign so it will not be used.
