- comments: 2-101. 11---observed no certified food manager on site or valid documentation while (tcs foods) (chicken, burgers, pizza etc. )are being prepared/served to the customers. Instructed person in charge, a certified food manager with city of chicago foodservice sanitation certificate must be present while preparing handling and or serving (tcs foods). Priority foundation violation 7-38-012.
38. Insects, rodents, & animals not present
- comments: 6-501. 111---observed approximately 50 mice droppings under dry storage racks and inbetween clutter articles stored in the rear. Instructed to clean and maintain. Suggested to contact pest control company to treat for rodents. Priority foundation violation 7-38-020(a).
