- comments: observed no pest control log book on the premises from a licensed pest control company. Management instructed to provide a pest control log book from a licensed pest control company with all required documentation (idph license, msds sheets, map of premises with bait and trap locations, etc. ). - comments: observed the 3-compartment sink at the back corner bar near the washrooms backing up into the other 2 compartments when the water is run in the left side compartment. Management instructed to repair and maintain the 3-compartment sink so that it drains properly. Priority violation 7-38-030(c). Citation issued.
