10. Adequate handwashing sinks properly supplied and accessible
- comments: no hot water available at the dish/prep hand washing sink. Hot handle stripped and unable to turn on and off. Missing cold water handle on the bar hand washing sink. A clamp being used to turn on and off the cold water. Instructed to provide hot water at prep/dish hand sink and replace the cold water handle on the bar hand washing sink. Priority violation 7-38-030(c) citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: cooks line pizza prep cooler unable to maintain adequate temperature. Cooler noted at 55. 4f. No foods inside the cooler at this time. Cooler tagged held for inspection and must not use until repaired, maintains a temperature of 41f or below and c. D. P. H contacted for tag removal. Priority violation 7-38-005.

36. Thermometers provided & accurate
- comments: missing internal thermometers inside pizza prep cooler and one door refrigerator. Must provide.

38. Insects, rodents, & animals not present
- comments: observed approximately over 75 mice droppings in the following areas: on clean dish storage shelving in the hallway between the kitchen and the bar, behind the prep table next to the basement stairwell, on the stairs leading to the basement, along baseboard behind the deep fryer. Instructed to remove all droppings, sanitize all affected areas and recommend having pest control co. Service for mice. Last service noted 9-10-25. Priority foundation violation 7-38-020(a) citation issued.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: soiled linens improperly stored in the dish machine scrapper sink. Must properly store in a sealed container.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: the interior of the one door refrigeration and freezer unit in the walkway between the bar and the kitchen in poor repair with exposed insulation and repaired with duct tape. Instructed to properly repair to be smooth and cleanable or replace.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: no chlorine chemical test strips on site to monitor sanitizing at the three compartment sink. Must provide. Priority foundation violation 7-38-005 citation issued.

49. Non-food/food contact surfaces clean
- comments: observed dirty dishes with encrusted food debris at the three compartment sink, dirty fryer basket at the deep fryer from two days prior service. Instructed to clean after each day to prevent pest harboring.

- comments: dripping/leaking pipe under the dish washing machine onto the floor. Instructed to repair.

- comments: noted holes and openings in the baseboard walls in the following areas: under and around the dish machine, under the dish room three compartment sink, along steps leading to the basement, behind the deep fryer on the cooks line. ----also hole noted in the bricks in the ceiling next to the exhaust hood. Instructed to seal all.

55. Physical facilities installed, maintained & clean
- comments: missing floor drain cover under the unused dish washing machine. Must replace.

60. Previous core violation corrected
- comments: previous core violations from report #2609531 on 12-20-24 not corrected. #37--observed bulk containers in the kitchen food prep area without a label. Instructed to provide the common name on all bulk containers to properly identify item (salt, sugar, flour etc). #49--instructed to detail clean tops/bottoms of prep tables,rolling utility carts, food storage containers,kitchen storage racks,interior and exterior of the freezer and prep coolers, stove and oven,and deep fryer also,storage shelves behind the front service counters and maintain. #55--instructed to repair broken corners of wall tiles next to the 3-compartment sink area so that the wall is smooth,easily cleanable and maintain. #55-: instructed to detail clean and organize basement and remove unnecessary items/articles/equipments from basement storage areas due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6" inches to facilitate cleaning. Priority foundation violation 7-42-090 citation issued.
