Details OBSERVED NO ONE ON SITE WITH A CITY OF CHICAGO CERTIFED FOOD MANAGER CERTIFICATE WHILE SHRIMP, CHICKEN AND BEEF ARE BEING COOKED AND SERVED. OBSERVED NO ORIGINAL CERTIFICATE POSTED. PRIORITY FOUNDATION VIOLATION 7-38-012
OBSERVED HANDWASHING SINK AT DISHWASHING AREA COMPLETLY OBSTRUCTED BY LARGE CONTAINERS AND BUS BUCKETS. PRIORITY FOUNDATION VIOLATION 7-38-030 (c)
OBSERVED WATERED DOWN DISH DETERGENT SUPPLIED IN BOWL AT HANDWASHING SINK FOR MAIN COOKING LINE. MANAGER WAS TOLD THIS WAS INADEQUATE AND INSTRUCTED TO USE HANDWASHING SOAP AT ALL TIMES. OBSERVED NO HANDWASHING SOAP AT HANDWASHING SINK IN DISHWASHING AREA. PRIORITY FOUNDATION VIOLATION 7-38-030 (c)
OBSERVED FOOD CONTAINERS BEHIND BAR THAT NEED LABEL. OBSERVED SPICES BEHIND COOKING LINE NEED IDENTIFYING LABELS.
OBSERVED 50 MICE DROPPINGS ON SECOND FLOOR DRY STORAGE AREA. OBSERVED 100 MICE DROPPINGS IN REAR EXIT AREA NEAR MOP SINK AND UNDER STAORAGE SHELVES. MUST ELIMINATE ALL DROPPINGS AND CLEAN AND SANTIZE ALL AFFECTED AREAS. PRIORITY FOUNDATION VIOLATON 7-38-020 (A)
OBSERVED INTERIOR OF ICE MACHINE WITH DIRT ON INSIDE. CLEAN AND MAINTAIN INTERIOR OF ICE MACHINE.
OBSERVED THE FOLLOWING VIOLATION NOT CORRECTED FROM REPORT # 2229355 DATED 10/4/18 #55 NOTED THE FOLLOWING-- FLOORS THROUGHOUT THE PREMISES ESPECIALLY ALONG WALL BASE AND UNDERNEATH COOKING EQUIPMENT WITH DIRT, GREASE AND FOOD DEBRIS. INSTRUCTED TO CLEAN AND SANITIZE AND MAINTAIN. INSTRUCTED TO DETAIL CLEAN AND MAINTAIN ALL COOKING EQUIPMENT ESPECIALLY THE COOKING STOVES ELECTRICAL COMPARTMENT OF THE DEEP FRYER. INSTRUCTED TO DETAIL CLEAN AND MAINTAIN #36 THERMOMETERS NEEDED INSIDE ALL REFRIGERATION UNITS. PRIORITY FOUNDATION VIOLATION 7-42-090
