- comments: no valid city of chicago certified manager on duty/site while tcs foods are being prepared and handled such as ground beef and cheese sauce for chips. Instructed manager that a city certified manager must be on duty at all time. Priority foundation violation #7-38-012.
3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: premises has no employee health policy as require by the new code. Instructed manager to comply or citations will follow. Priority foundation violation #7-38-010.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the hand wash sink in the prep area with a broken pipe underneath the sink. Instructed manager to repair and maintain. Also instructed manager to use the hand sink in the washroom. Priority violation #7-38-030(c).
38. Insects, rodents, & animals not present
- comments: observed the front door in poor repair(do not close at all), observed the rear exit door open: instructed manager to provide and maintain a screen when open to prevent pests entry point.
51. Plumbing installed; proper backflow devices
- comments: instructed manager to provide and maintain a back flow device on the rear mop sink.
55. Physical facilities installed, maintained & clean
- comments: floors in poor repair and dirty. Instructed manager that floors should be smooth and easily cleanable.
57. All food employees have food handler training
- comments: instruced manager to comply with the idph food handler requirement for all employee's handling tcs foods.
