Report date: Apr 10, 202622. Proper cold holding temperatures
- comments: noted internal temperatures of the following time/temperature control for safety food items inside the food display cooler to be improper: sliced ham at 48. 2 sliced tourkey 55. 4f, sliced corned beef at 57. 2, and 58. 2f, roasted turkey at 57. 2f, raw beef meat at 48. 4, boiled shrimp at 49. 9, pre-packed salmon at 49. 9f, sliced tomato at 54. 0f, diced tomato at 55. 2f, chicken breast 57. 7f, pre made tuna at 46. 5f, scrambled eggs at 45. 7f, variety of cheese at 56. 4f. Manager immediately and voluntarily disposed of 700 lbs of products worth $15,000. 00 through denaturing process. Priority violation #7-38-005. Citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: noted temperature of 3-door reach in prep display cooler to be improper at 62. 6f. Instructed not to use until equipment is maintaining proper temperature of 41f and below. Tagged and held for inspection by the chicago department of public health refrigerator unit must not be used until reinspected by chicago department of public health (cdph). Priority violation #7-38-005. Citation issued.

- comments: observed grab & go food items on display not properly labeled. Instructed person in charge to properly label all deli meats, re- packaged food items with identity, net weight, ingredients, list major allergens, business name and address where made and packaged. Priority foundation violation #7-38-005. Citation issued.
39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the prep and storage areas and throught out entire store. Instructed manager to properly store and protect food items from contamination while stored by elevating at least six inches from the floor on raised shelving units.

- comments: observed cutting boards in the rear prep area with open seams and dark stains. Instructed to refinish or replace damaged or stained cutting boards and maintain at all times.

- comments: observed milk, and soda crates, and empty boxes being used as shelving throughout the prep and storage areas. Must remove and provide shelving that is 6" off the floor and provides floor accessibility to ensure adequate facility cleaning.

- comments: observed torn door gasket, broken doors inside of the 2 door prep cooler in rear prep. Instructed to repair and replace gaskets

- comments: observed the interior of the reach in coolers in need of cleaning. Instructed to detail clean.

- comments: observed wood floor in rear prep in complete disrepair missing floor panels and broken floor panels instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed damaged wall tiles in the toilet room. Instructed to repair to create a smooth and easily cleanable surface. Must maintain at all times.

- comments: observed clutter stored on floor near in rear storage area. Instructed manager to remove all unused articles and equipment to prevent pest harborage.
60. Previous core violation corrected
- comments: noted previous core violations not corrected from report # 2625297 conducted on 10/14/25 (16) 4-602. 11(b-e): **c** equipment food-contact surfaces and utensils-frequency observed ice machine with a small amount a pink residue on the inside panel. Instructed to schedule a detailed cleaning and maintain. (#39) 3-307. 11: **c** miscellaneous sources of contamination observed two boxes of whole fruits and veggies (watermelon, cantaloupe, avacado) stored on the floor in the front of deli display. Instructed to elevate at six inches above the floor to protect from contamination. (51)5-205. 15: **c** system maintained in good repair (in good repair) observed three compartment sink faucet continously running when turned off. Instructed to repair and maintain. Must correct core violations by the date on the report priority foundation violation #7-42-090. Citation issued.
