- comments: observed evidence of appx. 15 or more live small flies on boxes and shelving units in dish washing area and appx. 30 or more mice droppings on lower shelving units at service counter and in toilet room on top of pressure washer. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 9/25/20. (priority foundation 7-38-020(a)

- comments: observed employees wearing soiled aprons. Must provide clean attire.

- comments: must clean grease and food debris build up from fryer cabinet,wok station & faucets,prep & reach in coolers,walk in cooler shelving units,exterior of food storage containers. Must defrost chest freezer.
