- comments: observed 1/4' gap on bottom right corner of exit door in frotn bar area. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: obsrerved over 60 mice droppings scattered on floor along walls and corners in electrical rooms, 40 mice droppings scattered on wooden ledges and floor in rear bar area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed ice buildup in walk-in freezer. Instructed manager to defrost freezer and maintain.

- comments: observed debris on interior and exterior surfaces of rolling carts, shelving units, cooking equipment, coolers, and freezers. Instructed manager to clean and maintain.

- comments: observed stained and missing ceiling tiles in storage areas, loose baseboards and openings along baseboards behind ice machine, damaged baseboards and wall tiles in prep area, openings around pipe fittings, prep and storage areas. Instructed manager to seal all openings in walls and ceilings, along baseboards, around pipe fittings in all areas.
