- comments: observed approximately 30-40 flies at small bar area. Flies were in 3 compartment sink, on walls and ceiling. Instructed to deep clean remove all standing water and contact pest control for addition service. Priority foundation 7-38-020(a) citation issued

- comments: observed an incorrect chemical test kit on site. Premises had a water testing kit on site. Instructed to obtain a quat test kit to be used with the quat sanitizer on site. Instructed to correct and maintain at all times. Priority foundation violation 7-38-005 citation issued

- comments: observered plates being air dried on rack above the 3 compartment sink. Rack in need of detail cleaning to remove all build up. Instructed to rewash utensils and clean rack. Correct and maintain.
