- comments: observed no clean up procedures on premises for vomiting and diarrheal events. Instructed to provide. Must provide all the necessary items in policy including disposable gloves, face mask, disposable mop with appropriate sanitizer for the norovirus. Priority foundation violation #7-38-005.

- comments: observed bottom area of rear not protected for rodent entry. Must repair door.

- comments: observed dry storage shelves, and shelves inside walk in cooler not clean of debris. Instructed to clean/maintain.
