- comments: no clean up supplies or procedures onsite for vomiting or diarrhea events. Instructed to provide supplies and post procedures for employees to follow. Priority foundation. 7-38-005

- comments: found approximately 20 rodent droppings in corners throughout the store, front corner near antacids, and rear corner near detergents. Instructed to remove droppings, clean and sanitize he affected areas, and contact pest control for service. Priority foundation violation 7-38-020(a)

- comments: found no record of the pest control company's illinois department of health license number. Instructed to contact pest company to obtain a copy of the license and maintain onsite. Priority foundation 7-38-020(c)

- comments: found no hot water in the mens or womens toilet rooms during the inspection. Instructed to contact plumber to have hot water restored to a minimum of 100f in handwashing sinks. Priority violation 7-38-005.

- comments: found no back flow preventer device attached to the faucet or waterline of the mop sink. Instructed to install and maintain.

- comments: found debris on floors throughout the rear storage/dock area, instructed to clean and elevate items 6' off floors to prevent rodent harborage.
