- comments: observed a debris (in corners) accumulation on the interior of the ice machine. Must remove debris, detail clean machines and maintain.

37. Food properly labeled; original container
- comments: must provide identifying labels on all food articles stored out of original containers with the common name of food, in english; and maintain at all times.

38. Insects, rodents, & animals not present
- comments: observed mice droppings underneath booths, under hot and cold holding buffet unit, along the walls and in all corners in dining and in prep/cooking area, inside cabinets, on storage and prep shelves and counters throughout (front display area, dining area, kitchen area, inside walk-in cooler). Instructed to detail clean, wash, rinse and sanitize the affected areas. Recommend to contact license pest control for another service. Priority violation 7-38-020 (a) no citation issued

49. Non-food/food contact surfaces clean
- comments: additional cleaning of food debris and dirt accumulation onto all the outside surfaces and the interiors of the cooking equipment, the storage shelving, the cabinets, counters, on food containers, inside the equipment, in the cashier area, inside refrigerators, inside walk-in cooler, inside the booths. Must detail clean and maintain.

- comments: observed the toilet room handwash sinks slow draining. Must repair and maintain.

- comments: observed the following areas of facility not clean: the ceiling tiles throughout the kitchen, the toilet rooms, and the hallways the wall bases throughout the light shields in the kitchen must repair and maintain facility.

- comments: additional cleaning on all the ventilation hoods, exhaust ventilation fans in the toilet rooms, the kitchen, and the dining area. Clean condenser fan cover inside walk-in cooler.
