2. City of chicago food service sanitation certificate
- comments: observed no individual holding a city of chicago foodservice manager certificate on site at time of inspection. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: repeat core violation. See violation #60

16. Food-contact surfaces: cleaned & sanitized
- comments: observed low temperature dish machine located in rear dish washing area maintaining an improper sanitizer concentration of 0ppm of chlorine. Machine tagged and held for inspection. Instructed to repair dish machine so that it maintains a minimum concentration of 50ppm chlorine at all times. During course of inspection an ecolab tech arrived on site and repaired machine. Priority violation 7-38-025. Citation issued.

23. Proper date marking and disposition
- comments: observed containers of prepped pork and beef in prep cooler and walk in cooler located in rear prep area date marked 7/11/2020. Management voluntarily discarded and denatured approximately 20lbs of product valued at approximately $100. Instructed management tcs foods prepped on site may not be kept longer than 7 days. Priority violation 7-38-005. Citation issued.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical test strips for determining sanitizer concentration on site at time of inspection. Instructed to maintain test strips on site at all times. Priority foundation violation 7-38-005. Citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed missing ceiling panel in ceiling of mop sink room. Instructed to repair and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed unguarded light bulbs in ceiling above mop sink room. Instructed to shield and maintain.

57. All food employees have food handler training
- comments: observed no food handler certificates on site at time of inspection. Instructed all employees must complete food handler training and maintain record thereof on site at all times.

58. Allergen training as required
- comments: observed no allergen training on site at time of inspection. Instructed any employee holding a city of chicago foodservice manager certificate must complete allergen training and maintain record thereof on site at all times.

60. Previous core violation corrected
- comments: observed repeat core violation from report #2301208 dated 7/16/2019 not corrected: "observed no hand washing sign or poster near hand washing sink in front prep area. Must provide and maintain. " instructed to install hand washing signage and maintain. Priority foundation violation 7-42-090. Citation issued.
