- comments: observed no written employee health policy on site. Instructed facility to establish an appropriate employee health policy and maintain with verifiable signed copies on site for all food employees. Priority foundation violation #7-38-010. Citation issued.

5. Procedures for responding to vomiting and diarrheal events
- comments: observed supplies for responding to vomiting or diarrheal events. Supplies must include adequate sanitizer that eliminates norovirus. Instructed facility to provide clean up policy and supplies to properly handle bodily fluid spills on site. Priority foundation violation #7-38-005. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the following sink not maintained, no hot or cold water a the hand sink in the restroom. Instructed to repair hand sink on premise. Priority violation #7-38-030(c). Citation issued.

- comments: noted internal temperatures of the following time/temperature control for safety food items inside the food display cooler to be improper: ground beef 47. 0f, italain beef 46. 7f, rice 46. 2f. Also observed improper in the prep cooler unit: sliced tomatoes 62. 0f, mozzarella cheese 48. 2, cucumber gyro sauce 54. 3, and eggs 52. 9f. Manager immediately and voluntarily disposed of 3lbs of meat products, 2lbs of salad, and 3lbs of rice worth $250. 00 through denaturing process. Priority violation #7-38-005. Citation issued. Cos

- comments: observed temperature of food prep cooler to be improper at 60. 1f. Instructed not to use until equipment is maintaining proper temperature of 41f and below. Tag was placed on display cooler. Priority violation #7-38-005. Citation issued. Cos

- comments: observed no thermometer on site to monitor the internal food temperature of tcs. Facility must obtain and maintain proper thermometers. Priority foundation violation #7-38-005. Citation issued

- comments: observed no thermometers in the 2-door display cooler and prep cooler. Instructed to provide conspicuous and easily readable thermometers inside all coolers units.

- comments: observed food stored in containers in all the 2 door display cooler without proper labeling of name. Instructed to person in charge to provide a label reflecting the food item's name when food has been remove from its original packaging.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit for sanitizer or for ware washing area. Informed person in charge a test kit that accurately measures the concentration in mg/l of sanitizing solution must be provided. Priority foundation violation #7-38-005. Citation issued.

49. Non-food/food contact surfaces clean
- comments: observed grease buildup behind cooking equipment (fryer), walls, and underneath of the fryer on the floors in prep areas. Instructed to degrease, clean and sanitize.
