- comments: pic doesn't have a food managers certificate. Must provide and maintain. (priority foundation)
2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs ready to eat foods are being prepared,handled and served such as (chicken,gravy,potatoes,etc) must be on site at all times. Certified food manager arrived on site at 1:40 p. M. (priority foundation 7-38-012)
38. Insects, rodents, & animals not present
- comments: observed an appx. '1/2-3/4' gap along bottom of south eadt door. Must make door tight fitting.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must repair or replace worn door gaskets on 2- door reach in freezer.

- comments: must clean debris build up from fryer and wheel bases,2- door reach in freezer.

- comments: must repair or replace leaky pipe along wall base near mop sink,missing top on back flow device at 3- compartment sink,leaky toilet at base of toilet.

- comments: must repair or replace stained ceiling tiles in dining and hallway outside of toilet room. Missing floor tiles where needed. Must grout floor in walk in cooler,prep & dish washing areas.
