Report date: May 12, 20205. Procedures for responding to vomiting and diarrheal events
- comments: observed no clean-up policy procedure and items for vomiting and diarrhea. Must provide and maintain. (priority foundation 7-38-005)
38. Insects, rodents, & animals not present
- comments: observed evidence of appx. 50 or more mice droppings on several shelving units and inside of boxes on rear storage shelves and on floor in office behind articles on floor. Must remove droppings. Clean and sanitize areas. Recommended to have a pest control operator to service premises on or after 5/12/20. (priority foundation 7-38-020(a)
38. Insects, rodents, & animals not present
- comments: observed an appx. '3/4-1' gap along bottom of rear door. Must make door tight fitting.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: must eliminate crates used for food storage and provide adequate shelving units.
49. Non-food/food contact surfaces clean
- comments: must clean debris build up from counter tops,lower cabinets,cappuccino machine,table top cooler,pop & food display shelves,unused equipment in rear storage area,window sills. Must defrost ice cream freezers and clean.
51. Plumbing installed; proper backflow devices
- comments: must replace missing toilet tank lids in both toilet rooms and caulk hand sink in same.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed all 3- outside garbage cans over flowing with trash and boxes and on ground in corral. Must remove trash & boxes,clean and maintain area. (priority foundation 7-38-020(c)
55. Physical facilities installed, maintained & clean
- comments: must repair or replace stained ceiling tiles throughout premises,damaged & holes in walls in vestibule and rear storage area.
