- comments: observed front door with gap at base of door. Must seal base of door to prevent pest entry.

- comments: observed aproximately 30 rodent droppings in storage side of facility 3944 n sheridan. Must remove all rodent droppings and clean and sanitize all affected areas. Must update log book to include both 3944-3946 n sheridan. Priority foundation 7-38-020(a)
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed raw wood walls around sinks and storage shelves in rear storage area of store. Observed raw wood floors in rear of store. Must paint or seal all raw wood so that it is non porous, smooth and easily cleanable.
54. Garbage & refuse properly disposed; facilities maintained
- comments: observed palettes and excess wood around outside garbage area. Outside garbage area must be kept clean and maintained.

- comments: observed mops need to be hung to dry after use.

- comments: observed cracked floor/broken floor tiles in rear of store. Must replace. Observed broken floor tiles in area behind register. Must replace and maintain.

- comments: observed rear room on 3944 side with excess clutter creating rodent harborage. Must get rid of all unnecessary items and store all remaining items at least 6 inches off floor.
