- comments: observed pic in charge has no. Instructed that pic must have a valid city of chicago food certificate priority foundation

2. City of chicago food service sanitation certificate
- comments: observed pic on duty at this time has no valid city of chicago food sanitation certificate posted. Tcs foods being prepared and handled ( reheating sauces etc, foods cook to order). Alfredo priority foundation issued 7-38-012

16. Food-contact surfaces: cleaned & sanitized
- comments: observed high temperature dish machine being used sanitizing at final rinse of 144. 0f. Instructed to repair. Must sanitize at final rinse of 180. 0f. Instructed to use additional dishmachine and 3- comparttment sink to clean/ sanitizing food equipment. Priority foundation issued 7-38-025

22. Proper cold holding temperatures
- comments: observed approx 1-lb of cup coleslaw at improper cold temperatures of 49. 0f, 50. 0f,3-lb cups of homemade ranch salad dressing at 50. 0f. Instructed to maintain cold ( tcs) foods at 41. 0f or below. Foods was removed and discarded by pic at cost approx $ 35. 00. Priority violation issued 7-38-005

33. Proper cooling methods used; adequate equipment for temperature control
- comments: obser ed cooler being used maintaining improper temperature of 50. 0f. Instructed that cooler must maintain 41. 0f or below. Instructed to use additional cooler thats 41. 0f or below-cos. Cooler will be tag held for inspection. Must contact health dept for tag removal. Priority violation issued 7-38-005

38. Insects, rodents, & animals not present
- comments: observed rear entry door in dishmachine area not rodent proof approx 1/4 inch at bottom areas. Instructed to repair to be tight-fitting.

- comments: observed slight build up on inside upper panel of ice machine. Instructed to claen. Maintain.

55. Physical facilities installed, maintained & clean
- comments: observed floors inside 2ndfl liquor storage room not clean of debris. Must clean/maintain.

- comments: observed no ventilation inside employees washroom. Instructed to install.
