- comments: noted dirt and grease accumulation inside, on pipes/conduits and on edges of the exhaust hood and filters above cooking equipment. Instructed to detail clean and maintain hood, filters and pipes/conduits. Wheels of cooking equipment with excess grease clean and maintain.

- comments: 4-501. 11 noted ice build up inside the walk in freezer. Instructed to repair and maintain the condensation units.

- comments: 5-205. 15 noted leaking faucet at 3 compartment sink. Instructed to repair and maintain.

- comments: hole on wall at the rear dry storage area. Instructed to repair and maintain above. Instructed to detail clean and sanitize floors throughout especially at the areas with the waste water backup. Also noted missing ceiling tiles at the dining area by the exit door. Instructed to replace missing tiles. Observed standing water between floor tiles in dishroom and throughout premises. Instructed to re-grout floor tiles with a smooth surface.
