10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at the exposed handsink near the refrigerator in the rear prep area. Instructed the person in charge to provide handwashing signage at all handwashing sinks used by food employees and maintain.

- comments: observed hand washing sinks throughout the kitchen preparation area not accessible. Observed plates and cups stored inside the hand sink and trash cans were blocking hands. Priority foundation violation 7-38-030(c), citation issued.

38. Insects, rodents, & animals not present
- comments: observed three live cockroaches on the wall behind the three-compartment sink in the rear warewashing area. Instructed the person in charge to remove roaches, clean surfaces, and maintain premises free of pest. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed pest control log book with reports from 04/02/21 - 10/13/21; no 2022 or 2023 service reports. Informed the person in charge that all retail food establishments must maintain a log book, which shall be kept for twelve (12) months and include a copy of the service tickets and the pest control company�s illinois department of public (idph) health license number. Instructed to provide. Priority foundation violation #7-38-020(c). Citation issued.

- comments: observed food items stored on the floor in the rear prep and dry storage areas. Instructed the person in charge to elevate all food six inches off the floor and maintain.

- comments: observed soiled wiping cloths stored on prep tables. Instructed the person in charge to store in sanitizing solution between uses and maintain.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed multiple cutting boards with deep scratches and stains on prep surfaces in the rear prep area. Instructed the person in charge to resurface/replace cutting boards and maintain.

- comments: observed foil used as liner on shelving units in the reach-in cooler near the front entrance. Instructed the person in charge to remove all foil from shelving units and maintain. All food and non-food contact surfaces must be smooth and easily cleanable.

- comments: observed accumulated grease and food debris on interior and exterior surfaces of cooking equipment, prep tables, coolers, freezers and shelving units. Instructed the person in charge to clean and maintain. (nonfood contact surfaces)

- comments: observed multiple damaged ceiling panels in the rear dry storage area. Instructed the person in charge to repair damaged ceiling panels and maintain.

- comments: observed grease build-up on the ventilation hood and filters above the cooking equipment. Instructed the person in charge to remove grease build-up from the ventilation hood and filters and maintain.

- comments: observed no food handler certificates on site. Instructed the person in charge to comply with idph food handler requirements.
