- comments: no original and valid city of chicago food service sanitation certificate posted nor provided on site for present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled. Priority foundation: 7-38-012, citation issued.

- comments: ice build-up on pipe at condenser unit inside the walk-in freezer, instructed to repair issue.

- comments: additional hot water flow is required at handwashing sink in the kitchen by the east three compartment sink. Also the three compartment sink at west area of the kitchen,once one faucet is turned on the other faucet at other sink(same sink) looses pressure. Repair issue. Faucets at three compartment sink( east wall) are leaking. Repair issue.

- comments: excess cigaret butts on ground at outdoor by the basement rear entrance door,also at patio were resident smokes. Front area along building. Clean and maintain.
