- comments: observed no person in charge present with city of chicago certified manager certificate.
2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on site while(tcs foods)chicken,beef,rice,gravy etc. Is being prepared/handled/served. Instructed person in charge,that a city of chicago certified food service manager must be on site at all times when (tcs foods) are prepared,handled, and/or served. Priority foundation violation 7-38-012,citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink in the kitchen food prep area not accessible for employees to wash hands. Observed storing kitchen bowls and spoons inside mentioned hand sink while food handler actively handling and cooking foods to customers. Instructed manager,handsink must be accessible for employees to wash hands at all times. Priority foundation violation 7-38-030(c),citation issued.

- comments: observed stained cutting board on top of main prep line cooler. Instructed to resurface or replace the cutting boards and maintain.

- comments: observed rust on the wire storage racks in the walk-in freezer. Instructed to remove rust and repaint or replace the shelves and maintain.

- comments: instructed to detail clean sides of cooking wok station and cooking stove to remove grease build-up and maintain.

- comments: observed dirty floors with dried-up debris spillage underneath the rear dry storage racks and walls with splashed food debris above the 3-compartment sink area. Instructed to clean floors and walls in detail and maintain.

- comments: instructed to detail clean the kitchen exhaust hood/filters to remove excessive grease build-up and maintain.

- comments: observed no certified food manager on site with food allergen training. Instructed manager to comply with requirement.
