- comments: observed evidence of rodent activity on the premises. Observed approximately 25 mouse droppings in the following areas: on the shelves containing drinks, bread, prepackaged pastries, on the windowsills, and on floor behind atm machine. Management instructed to clean and remove all rodent droppings from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation #7-38-020(a). Citation issued.

- comments: observed dead insects on shelves containing canned foods and on circuit breaker in rear above three-compartment sink. Instructed to remove, detail clean, sanitize, and maintain.

- comments: observed broken self-closing device on restroom in rear. Instructed to repair or replace self closing device and maintain at all times
55. Physical facilities installed, maintained & clean
- comments: observed clutter in rear near restroom and in rear near three compartment and utility sink. Iinstructed manager to remove all unused articles and equipment to prevent pest harborage.
