- comments: observed no hand soap provided at the women's and men's staff washrooms in the cac building. Management instructed to provide hand soap at all hand washing sinks at all times. Priority foundation violation 7-38-030(c).

- comments: observed no paper towels in the 3rd floor boy's washroom. Management instructed to provide paper towels at all hand washing sinks at all times. Priority foundation violation 7-38-030(c).
38. Insects, rodents, & animals not present
- comments: outer openings not protected. Observed double door entrance door, door 6 and pre-k double side door not rodent proof doors have approximately 1/2 inch opening on the bottom door. Must make doors tight fitting to prevent pest entry.

- comments: observed approx. 100 mouse droppings located in the 1st and 2nd floor lunchrooms and service areas. Observed approx. 10 mouse droppings inside of the yellow cabinets behind the 2nd floor food service line and approx. 30 mouse droppings inside of the yellow cabinets behind the 1st floor service line. Observed approx. 60 mouse droppings along the walls of the 1st floor lunchroom and inside of the radiators units. Management instructed to clean and remove all rodent droppings in all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a).

- comments: observed a dead mouse on a glueboard trap located under the radiator and along the wall in the 1st floor lunchroom. Management instructed to remove all dead rodents from the premises.

- comments: observed garbage and litter on the ground surrounding the garbage compactor. Management instructed to maintain the garbage area clean to prevent rodent harborage conditions. Priority foundation violation 7-38-020(b).

- comments: observed 1/2" gaps along the perimeter walls of the 1st floor lunchroom. Management instructed to seal all open gaps to eliminate rodent harborage conditions.
